What is the Thames Valley Family Health Team?
We are one of the largest Family Health Teams (FHT) in Ontario. With more than 120 physicians across 18 sites within London/Middlesex, Oxford and Elgin counties, we are committed to providing team-based primary health care to over 162,000 patients.

A FHT is exactly that – a team of health professionals working together to deliver the very best patient care.

Your FHT includes not only family physicians, but registered nurses, nurse practitioners, dietitians, social workers, pharmacists, occupational and respiratory therapists, and administrative staff working together.

As a rostered patient, you have access to all of those professionals through your physician’s office. You also have access to a wide variety of patient programs and workshops that we offer to help you manage chronic conditions and improve your physical and mental health.
